British Red Cross Hospital and Community Support Project
(BRC Includes the Home from Hospital Service and the Next Steps Reablement Service)
The Home from Hospital service is usually for older people and is designed to assist users with everyday activities following a stay in hospital or personal crisis in the community. Provides up to four home visits over a four to six week period giving assistance with tasks such as shopping and prescription collection, completing forms, going for walks and providing an escort for appointments. The Next Steps Reablement Service assists users to regain independence and re-learn skills which are essential for daily living. Up to eight home visits over a six week period helping with activities such as shopping, preparing meals, improving indoor and outdoor mobility, developing a routine for carrying out domestic chores and starting a local hobby or interest. Does not provide transport or nursing/personal care and alcohol or drug users or those with aggressive behaviour are disallowed.
