MOSAIC Integrated Service for Children and Young People With a Disability

(CNWL)

MOSAIC aims to support children and young people with disabilities, along with their families, with independence and involvement in all aspects of home, nursery, school, leisure and community activities.

The multi-disciplinary service includes:

Speech and language therapists
Occupational therapists
Physiotherapists
CAMHS professionals such child and adolescent psychologists and psychotherapists
Specialist health visitor
Paediatric dietitians
Paediatricians

MOSAIC includes the following services.

MOSAIC professionals are skilled in supporting the needs of children and young people with disabilities. The service aims to help children and young people to join in with all aspects of school, leisure and community activities, and support their independence and involvement in all decisions that affect them, preparing them for adult life.

The service provides assessment, diagnosis and therapeutic intervention from a range of professionals from health and social care to create a ‘team around the child’.

It aims to achieve this through:

Working with families to develop a greater understanding of their child’s needs and to agree a family support plan that is reviewed over time.
Making sure that appropriate services are available to all children, young people and families, based on their individual needs.
Listening to and learning from the experiences of children, young people and families who use the service.
Sharing specialist knowledge and skills with families, other organisations and the wider community.

Where is the service located and what areas does it cover?

The service is based in Kentish Town Health Centre and we work with the whole local authority area. Some of our services are delivered from Swiss Cottage School.

Who does your service provide for?

From 0 – 18 for children and young people with a disability, or those who require a multi-professional assessment to determine this. Children are eligible if they require a number of health professionals to deliver integrated care. This is usually 3 or more health care professionals, including a paediatrician. The service is part of the wider Camden Integrated Children's Service and works closely with schools teams and other community health teams.

How can a child or young person start using the service?

A referral needs to be made to the single Point of Referral (SPOR) by a professional that knows the child and family such as the health visitor, the child’s nursery or school or a health professional already working with the child. Some services such as the sleep service may require your child to have an existing open referral to another MOSAIC service.

How are decisions made about eligibility for your service?

A Single Point of Referral Panel meets fortnightly to decide the best pathway for all children referred that have a developmental concern or disability. The panel is made of children’s clinical leads for MOSAIC, for Speech and Language Therapy, for Occupational therapy, for Physiotherapy and for doctors. The person referring will receive a response to the referral they make and a copy is given to parents. If the referral is accepted to MOSAIC, a member of MOSAIC will contact the family and explain what will happen next.

How do you communicate with service users and how are they involved in decision making/planning?

We will send you letters, and emails, and will call you at points throughout the process. All professionals in MOSAIC work with interpreters when needed to ensure that everything about the family’s child’s needs are understood by the family. We may ask you to complete questionnaires from time to time. British Sign Language signers are available. Many of the staff in MOSAIC are trained in other communication methods with children such as Makaton and Picture exchange Communication System. Is your service fully accessible?

The building is fully wheelchair accessible. The service has been designed with parents however there are no specific adaptations for hearing and vision. There are disabled changing and toilet facilities. The Play and Wait reception area on the first floor is child centred in its design and equipment available. The services offered will be specifically to meet the child’s individual needs and will be accessible.

What training have the staff supporting children and young people with SEND had/or will be they be getting?

All staff in MOSAIC have received specialist training in a range of disabilities such as Autism, Cerebral Palsy, Down Syndrome and Learning Disability and will continue to take up relevant training to assess and support children with disability and their families. Many who work in the service are also trained in Makaton and PECS (Picture Exchange Communication System). Staff have a very high level of understanding about the impact on the family of caring for a disabled child.

A parent / carer who wishes to raise a concern or make a complaint should be encouraged firstly to discuss this with the professionals working with the family or the team leader.
