Life Force Team

(Camden Haringey and Islington Paediatric Palliative Care and Bereavement Service)

We are a multi-disciplinary team that support children with life limiting/life threatening conditions. We support children living in Camden, Haringey and Islington. The team consists of:

  • Community Matron, who manages the team, has a caseload of her own and works strategically to raise the profile of paediatric palliative care.
  • Working alongside the Community Matron are specialist nurses who provide training and support to the respite nursery nurses, support to the parents and the child and end of life care in the home.
  • The specialist nurses also provide bereavement support to the parents and debriefs to staff.
  • Short Breaks (Respite) nursery nurses who provide sessions of short breaks in the home allowing parents to leave the home. This may be part of a joint short breaks package with MOSAIC social care team.
  • Play specialist/youth worker who works with the child to explore how they feel. She also provides a bereavement programme for the bereaved siblings.
  • A psychologist who provides pre and post- bereavement support to parents and clinical supervision to colleagues.

Where is the service located and what areas does it cover?

We work in the boroughs of Camden, Haringey & Islington. Our team is based in Northern Health Centre but we work with the children and young people in their homes.

Who does your service provide for?

We look after children from 0 – 18 (up to 19 if they have special needs) who have a life limiting or life threatening condition.

How can a child or young person start using the service?

Families can make a direct referral. We have no waiting list. We are funded by the Clinical Commissioning Group and so there is no charge to families for our service. Sometimes the child or young person will be known to MOSAIC, the integrated service for disabled children and young people and a joint plan with MOSAIC would be put in place.

How are decisions made about eligibility for your service?

Referrals are discussed as a team. An assessment of needs is made and we communicate directly with the families as to whether they are eligible for the service or not.

How do you communicate with service users and how are they involved in decision making/planning?

Children who are on our caseload long-term are reviewed every three months. We review the other children on a regular basis, at least monthly. We undertake a patient survey every six months to gather feedback about our service. We are currently using the National Patient Toolkit for this. We have a leaflet for parents and one for professionals. We actively use interpreters for non-English speaking families.

Is your service fully accessible?

The service is delivered to children in their home.

What training have the staff supporting children and young people with SEND had/or will be they be getting?

Our staff have all had training in Makaton (simple sign language) and disability awareness.
