Rebuilding Bridges - Count me in

About the service:

  • Inclusive youth provision (ages 13 – 25) supporting young people with autism, LD and SEMH to access universal youth services. This can include time-limited 1:1 support (usually up to 10 sessions), and some additional support to access holiday activities.
  • Count Me In runs an inclusive club on Monday and Thursday evenings at Somers Town Youth Centre. Young people may be supported to access other youth provision if this is more appropriate for their individual needs.
  • Provides support for young people to build up the confidence and skills to attend local activities and groups independently over time.
  • Count Me In can also give access to careers advice and employment support services.
  • Eligibility criteria 1 - Primary need:
    • Autism
    • Learning Difficulty/Disability
    • Social, Emotional and Mental Health (SEMH) needs
  • Eligibility criteria 2 - Presenting issues:
    • Lack of access/engagement with universal services
    • Social isolation / lack of peer support
    • Display behaviours of concern
    • Not in education, employment or training (NEET) / at risk of becoming NEET
    • Not engaging with/avoiding school/college
    • At risk of exclusion 

Priority will be given to children/young people who are:

  • Not eligible for enhanced Short Breaks (those receiving Short Breaks core offer can be considered if no other provision is appropriate to meeting need).
  • Not currently receiving ongoing support from CAMHS/EPS.
  • Not accessing specialist education provision (e.g. CCfL, Swiss Cottage) although “short-term” specialist provision will be considered (e.g. CRiB, Thomas Group)
  • Exceptional circumstances will be considered by the Count Me In team, depending on risks to child and young person if a service is not provided

  • Community & voluntary sector partners
  • Parents/carers

Referral process:

  • Referrals sent to Integrated Youth Support Service (IYSS) who will make decisions on resource allocation during weekly Count Me In team meetings. Referrals that do not meet the criteria for this programme may be able to access Youth, Connexions and youth Early Help services.
  • Parents/carers and referrers will be made aware of the outcome of panel allocations by LB Camden, including any alternative support that may be available if the referral is not accepted.
  • Once a referral has been accepted, the Count Me In team will contact the family to discuss next steps.

How to refer

  • Consent to engage with the project prior to making a referral must be obtained from the parent/carer (and young person if aged 16+) by completing a consent form.
  • Complete a referral form and send to the contact details below. Please ensure the referral form is completed with as much detail as possible. We will not accept incomplete referrals.
