MOSAIC (Making Our Services All Integrated in Camden) is the integrated service for disabled children, young people and their families from birth to 18 years of age, providing assessment, therapy and family support, including Short Breaks. The service provides Multi agency assessment, diagnosis and support for disabled children and their families from 0-18. The service contributes to better outcomes by supporting families to care for their disabled children and for their children to remain living at home and as part of their local community wherever possible.
MOSAIC consists of several services including:
- Child Development Team
- Complex Feeding Clinic
- Sleep service
- Social Communication Assessment Service
- Child and Adolescent Mental Health Services (CAMHS)
- EarlyBird and EarlyBird Plus
- Integrated Care Support Team
Help and support is offered early on from the point of referral whatever the child’s age is. The national Early Support model is offered. We offer a Keyworker to help with the co-ordination of all professionals working with the family so they work as a Team Around the Child (TAC), and one of the team will operate as the Assessment Co-ordinator for a child undergoing statutory assessment for an Education, Health & Care Plan.
Diagnosis and therapies are provided for disabled children following assessment and this includes children with Autistic Spectrum Disorder. Access to specialist clinics for disabled children with sleep difficulties and complex feeding difficulties are available. Emotional and behavioural support is provided for those families with a disabled child who would find it helpful. Social care, including Short Breaks as well as the protection of disabled children is provided. Access to parent / young people’s groups (Special Parents Forum, Reactive Forum), information and quarterly Newsletters is available.
