Home Based Learning is a service for pre-school children with a disability or developmental delay. The Portage trained Practitioner visits the child weekly at home to plan activities that will help with the child’s development. We aim to look at what the child needs and can do, and in partnership with parents/carers, plan fun activities that will take the child onto the next step of his or her development. - Where is the service located and what areas does it cover?
Our Portage Home Based learning service covers the whole of Camden, children need to have a registered home address in Camden to recieve the service. Developmental stay and play sessions take place at various locations in Camden contact Karen for more information
- Who does your service provide for?
Our service provides for families who have a pre-school child with a disability and or complex needs and reside in the borough of Camden.
- How can a child or young person start using the service?
The child should be known to Camden MOSAIC, the integrated service for disabled children and be referred to our service by them. Referrals are made to MOSAIC through a single point of referral (SPOR).
- How are decisions made about eligibility for your service?
The service is provided to children under five who are not in an early years setting or have a part time place. After receiving a referral from MOSAIC Child Development Team an appointment will be made, to visit the family at home. The KIDS Home Learning Coordinator will talk to the family and discuss the child’s needs with them. If both KIDS and the family feel this would benefit their child then a Portage practitioner will be allocated to the family.
- How do you communicate with service users and how are they involved in decision making/planning?
When a practitioner works with the family they will listen to the family’s concerns about their child and together with the practitioner’s observations of the child will decide on developmental goals and plan activities around these together. At the end of each visit the Practitioner will leave a written report (activity sheet) for the family to keep and refer to. Parents/carers are also given an end of service questionnaire so that they are able to comment on their experience of receiving home learning for their child.
- Is your service fully accessible?
Our office is fully accessible. However, we would usually visit children in their own homes.
- What training have the staff supporting children and young people with SEND had/or will be they be getting?
All practitioners are PORTAGE trained and have Early years training as well as a good knowledge of child development and working with children with disabilities. They have regular ongoing training as appropriate to their role. Staff all attend safeguarding training.
- Who should a service user contact if they want to raise a concern or complain about something?
