The Transformations Team is an integrated team providing early help and preventative, holistic support to whole families with children aged 0-19 who are experiencing multiple, competing, and interconnected stressors.
We provide bespoke and flexible approach to help build families’ resilience, aimed at making lasting changes to the lives of families at risk of the poorest outcomes, and improve outcomes for younger generations, through child-centred and family focused practical and evidence-based interventions.
The allocated primary worker provides support to all individuals in the home, based on their presenting needs in order to shift the trajectory and intergenerational patterns for the children.
We take a partnership approach to integrated care planning and delivery of services that actively involves families. The model supports a systemic practice based approach to both family and multi-agency working: streamlining and sequencing interventions so that fewer professionals compete for engagement with the family. Interventions are aimed at making lasting changes to the lives of families at risk of the poorest outcomes.
We provide direct, intensive work with families (children and adults) which lasts on average 12 months +; assuming the role of lead professional and coordinating Team Around the Family; information, advice and consultation for families and other professionals from specialists within the team, including DVA, Educational Psychology, Systemic Family Therapy, Psychotherapists, Child and Adolescent Psychiatrist, School inclusion, Probation, GP, Police, Employment advisors from DWP, Community safety and housing.
Referral to the service is through the Children & Families Contact Team.
